Go to the Account Activation page
You will be prompted to enter your “generated username, password, and DOB”.
Provide your username, password, confirm password, and date of birth (mm/ dd/ yyyy).
Click on the “Submit” button.

You can get access to your ‘Profile’, ‘Enhanced Communication’, ‘Clinical Summaries’, ‘Allergies’, ‘Medications’, and ‘Lab Test Results’.

Visit the login page of Your Health File
Enter your login details – username and password.
Click on the “Log In” button.

How to Retrieve Your Health File Username or Password?

Go to the login page of Your Health File.
Look for the option that says “Forgot username or password?”
Click on the option “Forgot username or password?”
Select what you have forgotten – username or
If you have forgotten “Username”, click on it. If you have entered the wrong password, select “Password”.
Enter the Date of Birth.
Click on the button “Retrieve Info”
